Job description Avanti Home Care's Registered Nurse (RN) Case Managers implement, evaluate and supervise the care of clients in their own homes. Avanti Home Care RN’s work as a collaborative part of the health care team. Providing total client care to include; assessments, organizing caseload, coordination of client records, oversees field staff, medications and treatments administered as ordered by the physician, physical and psychological care, and client and family education. Assesses and interprets symptoms and reports significant changes in client's condition. Qualifications:

• Familiarity with medical software and equipment, and high comfort level with computers and other forms of technology
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
• Ability to self-direct, self-schedule, and work independently
