JOB SUMMARY:

Responsible for providing home care services consistent with the mission, vision, and goals of the Agency. The RN is responsible for providing skilled nursing services under an interdisciplinary plan of care approved by the patient’s physician. Nursing care includes, but is not limited to, ongoing interdisciplinary patient assessment, evaluation, and coordination of the medical / nursing needs of the patient; performance of prescribed medical treatments; and instruction of caregivers in the provision of care to the patient. The RN shall provide for the continuity of services for the patient and his/her family.

DUTIES AND RESPONSIBILITIES:
  • Complete initial and on-going interdisciplinary comprehensive assessments of the patient’s needs including OASIS assessments at appropriate time points.
  • In partnership with the patient, representative (if any), and caregiver(s) identifies the physical, psychosocial, environmental, safety and developmental needs and initiates an individualized patient plan of care based on the needs identified.
  • Revise the plan of care as necessary in partnership with the patient, representative (if any), and caregiver(s).
  • Provide service and /or treatments requiring substantial and specialized nursing skills as ordered by the physician as indicated in the plan of care.
  • Initiate appropriate preventative and rehabilitative nursing procedures.
  • Counsel, support and instruct, and educate the patient, family and caregiver(s) in meeting nursing and related needs.
  • Refer to other services as needed.
  • Prepares clinical notes for the documentation of nursing assessments, problems identified, appropriate goals, care plan, interventions provided, and evaluation of patient / family outcomes achieved.
  • Coordinate all patient services.
  • Communicates with all physicians involved in the plan of care, other health care practitioners (as appropriate), and appropriate members of the interdisciplinary team related to the current plan of care and changes in the patient’s needs / condition.
  • Support the patient’s and family’s cultural beliefs.
  • Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance.
  • Demonstrate the ability to correctly use and direct the use of medical supplies and equipment.
  • Supervise care given by the para-professional, Home Health Aide, and Licensed Vocational Nurse providing services to the patient as required by law and licensure. Supervision is documented on the Nursing Visit Note.
  • Assign Home Health Aide (HHA) to a specific patient and prepare Assignment sheet.
  • Plan for discharge from service.
  • Participate in the On-Call rotation as requested.
  • Participate in orientation, in-service education, utilization review and patient care conferences.
  • Prepare and submit, as directed, all case documentation, including clinical and progress notes, and required payroll information according to Agency policy.
  • Provide services within the Agency's catchment area.
  • Acknowledge patient rights including privacy and confidentiality.
  • Consistently display a professional appearance.
  • Interact with internal and external customers using a positive and professional approach.
  • Assume responsibility and display reliability for the completion of all tasks, duties, communications and actions.
  • Support the mission, vision, and goals of the Agency.
  • Maintain professional growth and development.
  • Participate in the Agency’s QAPI program for performance improvement activities.
  • Abide by Agency policies and procedures.
  • Accept other duties as assigned.

 

EDUCATION, EXPERIENCE, AND TRAINING :
  • Graduate of an approved school of professional nursing.
  • Current, active license in the State of California by the Board of Registered Nursing.
  • Current CPR, health clearance and clinical skills self-assessment.
  • Successful passage (80%) of Agency competency exam and core competency per skills checklist.
  • Minimum of one year of professional nursing experience.
