TITLE: PACT Nurse
PROGRAM: Program of Assertive Community Treatment (PACT)
SUPERVISOR: PACT Nurse
MINIMUM QUALIFICATIONS:
Registered Nurse State of MA; Experience working with consumers with major mental illness and co-occurring substance abuse disorders, working as part of a multi-disciplinary team required. Experience working with individuals with a serious and persistent mental illness in the community preferred. Valid driver’s license required. Skilled in the use of Microsoft Word.
OVERVIEW:
Provides recovery-oriented treatment services to team consumers and performs indirect services as detailed below. Staff may be required to attend additional training on a range of topics such as evidence based practices, recovery-oriented treatment, trauma informed care, illness-management-recovery, and forensic specialties. Provides medication and health services. Maintains a caseload of consumers receiving outreach services.
Shifts - Sunday through Thursday or Tuesday through Saturday schedule.
RESPONSIBILITIES:
A. Coverage:
1. Provides outreach coverage as scheduled including weekdays, weekends and holidays.
2. Remains available to work extra hours in the event of emergency.
3. Remains awake and alert at all times while on duty.
B. Consumer Services
1. Promotes consumer recovery and wellness in all actions.
2. Establishes and maintains therapeutic, supportive relationships with all team consumers.
3. Assists consumers in identifying their personal recovery goals
4. Actively works with consumers to develop recovery-oriented, person-centered individualized action plans (IAP), as assigned.
5. Involves consumer’s Legally Authorized Representatives in planning
